A 50 year old with no past medical history presenting with the symptoms, a classic CXR and the expected blood results is 99% COVID-19.

An 80 year old with several comorbidities including severe COPD is more complex - for example, it’s difficult to tell from the scans and bloods whether it’s only a bacterial pneumonia that’s causing the increased O2 requirement, or whether there’s a super-added viral infection that’s kicked it off.

Thanks for this, I understand alot better now, I'm just surprised by the numbers in the group of 80 year olds (or similar) with symptoms falling into line with needing a test, and then not having it, it's an eye opener.

May be applicable for much of the region, although Turkey, Israel, Lebanon and the Gulf states appear to be relatively transparent (Turkey after reporting nothing for ages) while the likes of Syria, Iraq and Yemen may be a little too chaotic right now for any kind of accurate surveying.

I can confirm Syria is in a very weird state, while both my parents are doctors and haven't encountered any covid19 patients, the government shutting down the whole country basically and I think an Emirates prince said he stands with Syria in this tough situation casts big doubts over what the government is letting out.

I don't think it will be a surprise if significant social unrest emerges in the next week or two. Realistically, I think you can only have these types of measures in place for 6-8 weeks at most in a western country before it begins to unravel.

Unless you are a communist state like China, even when its for the greater good, you can only impose a very strict lockdown for a certain amount of time before cracks begin to show - especially if people are running low on supplies/money. Italy has been on the front lines of this far longer than the UK, i'm not surprised to see tensions rising.

Whether this particular piece of reporting is sensationalist or not, the social unrest it's describing is inevitable once a heavy lockdown reaches a certain point and would get worse beyond that as the lockdown continues. Whether or not Italy really is at that point now, it eventually will be, as will any other country that implements such measures.

Which is one of the reasons multiple countries repeatedly warned of the dangers of implementing these measures too early. They can only last so long before they break down, they can only last for so long before people start reacting against them and you'll struggle to get the same buy-in if you have to implement them a second time. So it really has to count for the limited amount of time you can sustain it.
