In the US everyone says East Indian for the sub continent, West Indian for the Caribbean and Indian for the first peoples. The use is fairly common in the US and standardized for my brothers there. The American Indian Movement would change its name if there was any perceived issue.
Different story in Canada, mind you, but most of us up here don't care that much. We've been called a lot worse.
