Yes, any player knocked out.

And no - you aren't swapping players, just adding to your squad. So you already have all 12 criteria met so don't need to worry about that. Just need to make sure your 11 players on the field meet 11 criteria.

This is where having diversity in your original squad helps, and picking in such a way you can make improvements while meeting 11'

You're really unlucky that Romario didn't win the Ballon d'Or as a straight swap for Weah. He would have been lovely in your team ahead of Raul.

Good team, pretty solid throughout.

Have you considered putting Nedved on the left and Bale through the middle? You would be getting Bale pretty close to his best position, and Nedved with Evra overlapping him would probably be just as potent as him playing through the middle.

Yes, I thought about that. Nedved was great at the left and would handle playing in a 4 man midfield. Also Bale getting a better position like his late Spurs years. Will have to think about it.

It also depende on what players I come up against.

Maybe if you get drawn against a team with a notably slow defender just have Bale stand near him?
