People going without a Hajj Visa was a big problem this year and that seems to be one of the main causes for the high number of deaths.

There two main reasons and they can be fixed easily. Countries that have automatic visas to Saudi was the biggest problem because the number of people who came without Haj visa was huge. They did not only cause more numbers that caused more pressure during the ceremonies. But also they did not have places to sleep and literally died of heat because they slept outside in the wild.
Two, the bottle neck is the (stones throwing) that occur for the millions at the same time and at the same spot. For Shia Muslims they do not need to do it at a specific time, but through the whole day which can help spread the numbers through out the day, but for the majority sunni muslims they have to do it at the same time which is behind most of the accidents.
